Report when a visitor starts filling in a Hellotext form - #104

On the first input, the form sends form.started with its id, once per session, form and day, for the website form report's "started filling it in" stage. Focus alone doesn't count, because the form focuses its first field when it connects. The daily mark is written only on a successful report, so a failed one is sent again on the next input. Tracking errors never reach the form. The same controller runs embedded forms and Hellotext's own form page, so both report. Rebuilt lib and dist.
